Please do not send medical details through the form. We will talk about your health in person or by phone.
New Patient Forms
Our new patient forms are a PDF you download, fill out offline, and bring to your visit. They are not submitted through this website.
We do not use advertising or tracking pixels, and we do not use website analytics.
Your Health Information
Protected health information in your patient records is handled under our HIPAA Notice of Privacy Practices, available at the front desk.
Sharing
We do not sell your information. We share it only with service providers that help us run this website and deliver your request, or when the law requires it.
